Support Coordination Services

Our Support Coordinators are there to support people living with disability to exercise their Choice & Control and be the decision-maker about their life.

​

We work with you to explore how you can live a full, inclusive and independent life,

in your community, your way.

​

   Support Coordination includes:

  • Assistance to understand your NDIS plan and available services under your funding

  • Connecting you with appropriate service providers and community supports

  • Monitoring service delivery to ensure your needs are met

  • Addressing barriers to accessing services and troubleshooting issues

  • Access to the right services if you are experiencing crisis

  • Preparing progress reports for the NDIA (if required) and support during plan reassessments
